- The Occupational Safety and Health Administration has charged that Twin Pines Construction Inc. of Everett, Mass., had multiple and, in one case, repeated fall-hazard violations at building sites where it was erecting trusses and framing exterior walls.
- The agency has proposed a total fine of $336,200 in fines.
- The investigation got started in March after a worker at a Plymouth, Mass., project where an unbraced roof truss system collapsed around a worker, who had leg injuries and broken ribs.

Inspectors from the OSHA office in Braintree, Mass., south of Boston, clearly were unhappy when they found unbraced trusses and potential falls of as much as 12 feet onto uncovered anchor bolts and records showed Twin Pines had been cited for the same issues in 2009 and 2011. Inspectors from a different office said they found workers on a second job did not have protection against falling 10 to 20 feet.
