• Schindler Elevator Corp. has received notice that the California Department of Occupational Health and Safety is reopening its investigation into the June 11 death of a technician at the construction site of Levi's Stadium.
• Don White, a mechanic for Schindler, was killed by a counterweight's fall at the Santa Clara stadium where the NFL's 49ers will play football beginning next year.
• The notice was dated Oct. 14, the same day that the $1.2 billion project suffered its second fatal accident of the year, the death of truck driver Edward Lake Jr. when a load of rebar fell as it was being unloaded from his truck.
